Once accepted into the program, the following documents must be completed and returned to Community Care Preschool prior to the start date:

- Consent for Medical Treatment (LIC 627)
- Parent’s Report (LIC 702)
- Personal Rights (LIC 613A)
- Identification and Emergency Information (LIC 700)
- Parent’s Rights (LIC 995)
- Physician’s Report – including immunization record (LIC 701)
